Foot beforehand does imply it was foot first though or the joke doesn't work.

For the numbness joke to work it makes no difference whether it was the foot before the hand or not. In fact the could have made his own free choice of using the foot or the hand if he was that dexterous.

Apparently you sit on your wrong hand i.e left if you are right handed until it goes numb wear a marigold glove and reach under your thigh to perform the task.
